A Case of Posterior Scleritis in Children with Ocular Pain and Visual Disturbance

Abstract
Purpose
We report a case of posterior scleritis in children with ocular pain and visual disturbance. Methods: A 13-year-old female patient visited our clinic with ocular pain in her left eye, which started a week earlier. The best corrected visual acuity (BCVA) was 1.0 in the right eye and 0.8 in the left eye. We observed conjuctival injection, episcleral vein enlargement in the left eye and fluorescence leakage in the left optic disc. Therefore we suspected left optic neuritis and episcleritis. After high dose corticosteroid therapy, the symptoms were relieved. Two weeks later, left ocular pain recurred and BCVA was decreased to 0.2 in the left eye. Fundus examination showed macular striae, multiple yellowish spots and exdudative retinal detachment. Gross thickening of the posterior sclera and edema of Tenon's capsule were identified in the B-scan. Therefore we diagnosed left posterior scleritis. Results: After oral administrations of nonsteroidal anti-inflammatory drugs and corticosteroids, BCVA in the left eye improved to 1.0 and exudative retinal detachment was disappeared. Recurrent episode occurred once after 10 months later, but relieved with same regimens. Conclusions: We experienced a case of the posterior scleritis in children with exudative retinal detachment which treated effectively with oral administrations of nonsteroidal anti-inflammatory drugs and corticosteroids.
