Abstract
Purpose: To report a case of a self-mutilation of his eyeball and eyelid.
Case summary: A 47-year-old male with paranoid schizophrenia attempted to tear out his eyelid and eyeball by hand tied up with restraining band. He had already sustained self-inflicted eyelid laceration using a pen. Emergency surgical exploration was done under general anesthesia. There were avulsive injuries of upper and lower eyelid. Canalicular and 360-degree conjuntival lacerations were present. Four extraocular muscles were ruptured and retinal hemorrhage was noticed. Primary wound repair was performed.
Conclusions: Autoenucleation can be one of the Self-harm of patient with Psychiatric disorder. Psychiatric referral for self-harm and close observation are mandatory even after operation, due to the possibility of repeat episodes and the increased risk of suicide.
