The Effect of Restasis on Tear Osmolarity Measured by TearLabTM in Dry Eye Disease

Abstract
Purpose
To evaluate the effect of 0.05% cyclosporine emulsion on tear osmolarity measured by TearLabTM (OccuLogix, San Diego, CA, USA) in dry eye syndrome. Methods: From patients diagnosed with dry eye syndrome, 0.05% cyclosporine emulsion and lubricant eye drop were administered to 100 eyes of 50 patients. The prospective study was enrolled with 44 eyes of 22 patients who had been performed with laser ephithelial keratomileusis (LASEK Group) previously and 56 eyes of 28 patients with no previous ocular surgical history (Non LASEK Group). From each patient, symptoms questionnaire based on ocular surface disease index (OSDI), Schirmer test with anesthesia, Tear film break up time (TBUT) and osmolarity were evaluated. The clinical parameters and symptoms were checked before and 1, 2, 3, and 6 months after treatment. Results: Ocular symptoms evaluated by OSDI symptoms questionnaire in LASEK and Non-LASEK group started to improve from 1 month after use of Restasis (Allergan Inc., Irvine, CA, USA) and Schirmer test, TBUT and osmolarity improved significantly (p<0.05). The changes of average values with time had a high correlation from 0.586 to 0.993 (p<0.05). Both OSDI and Tear osmolarity values decreased, and Schirmer test and TBUT values increased in each two group. The average values of tear osmolarity were statistically different at 1 month, 2 months and 6 months after administration of Restasis between two groups. Conclusions: The measurement of osmolarity of tear film can be used not only to diagnose the dry eye diease, but also to evaluate the therapeutic effect of Restasis as more objective and reproducible tests.
