Abstract

목적

아데노바이러스는 유행성 각결막염의 가장 흔한 원인이며 60개가 넘는 유전형 중 유행성 각결막염과 관련된 아데노바이러스는 혈청형 D와 관련이 있다. 하지만 한국의 아데노바이러스에 의한 각결막염의 유전형 및 그에 따른 임상 양상의 분석에 대한 연구는 아직 미흡한 실정이다. 이 연구의 목적은 한국에서 유행성 각결막염을 일으키는 주된 아데노바이러스의 유전형을 확인하고 그 임상 증상을 확인하기 위함이다.대상과

방법

2016년 10월부터 2018년 3월까지 임상적으로 아데노바이러스 각결막염이 의심되는 환자들에게서 20개의 결막도찰검체를 얻었다. 핵산은 QIAamp Viral mini kit를 사용하여 추출하였으며 QIAcube platform으로 핵산분석을 하였다. 아데노바이러스 capsid hexon 유전자에 특이적으로 유전자분석을 하였으며 이를 위해 중합효소연쇄반응(polymerase chain reaction)과 염기서열분석이 사용되었다.

결과

20개의 검체를 얻었으며 이 중 17예의 검체(85.0%)가 유전자분석이 이루어졌다. 11예의 검체(64.7%)는 아데노바이러스 8형, 5예의 검체(29.4%)는 37형, 1예의 검체가 64형으로 확인되었다. 유전분석이 이루어진 17예의 환자들은 모두 결막충혈(100%), 여포(100%)가 나타났으며 12예(84.6%)의 환자들은 가성막이 형성되었다. 다른 혈청형에 비하여 8형의 경우 각막을 침범하는 경우가 많았으며, 37형은 각막을 침범하는 경우 모두 실모양각막염의 형태를 보였다.

결론

연구 시기 동안 한국에서 가장 흔하게 확인되는 각결막염의 원인이 되는 아데노바이러스의 유전형은 8형, 37형이었다. 이 유전형의 주된 임상 증상으로는 결막충혈, 여포형성 및 가성막 형성이었다.

Trans Abstract

Purpose

Human adenoviruses are a major cause of epidemic keratoconjunctivitis. Of over 60 genotypes, subgroup D is associated with epidemic keratoconjunctivitis. There are, however, no data regarding the genotypes of conjunctivitis-causing adenoviruses in Korea. The purpose of this study was to determine the types of adenoviruses causing epidemic keratoconjunctivitis and clinical manifestations in Korea.

Methods

From November 2016 to September 2017, 20 conjunctival swab specimens were collected from patients who were clinically suspected to have adenoviral keratoconjunctivitis. Nucleic acids were extracted using a QIAamp Viral mini kit and processed on the QIAcube platform. Genotyping targeted for adenovirus capsid hexon genes was performed by using the polymerase chain reaction and sequencing techniques. Patient ages ranged from 18 days to 69 years (median, 37 years).

Results

Of 20 collected specimens, 17 (85.0%) were genotyped. Three samples were not amplified with our primer sets. Eleven samples (64.7%) were adenovirus type 8, five (29.4%) were type 37, and one was type 64. All 17 genotyped patients showed conjunctival injections (100%) and follicles (100%), and 12 (84.6%) showed pseudomembrane formation. In case of patients genotyped type 8, most of them showed corneal manifestations and involvement of cornea by type 37 showed filamentary keratitis.

Conclusions

This study showed that the polymerase chain reaction and direct sequencing for adenovirus hexon genes of conjunctival swab samples were effective techniques for genotyping the adenoviruses that cause epidemic keratoconjunctivitis. The most common causes of adenoviral keratoconjunctivitis in Korea were adenovirus types 8 and 37 in this period of time. Conjunctival injection, follicle formation, and pseudomembrane formation were the major manifestations of adenovirus types 8 and 37 infections.

아데노바이러스(human adenoviruses) 감염은 위장관염, 급성 호흡기 질환, 각결막염, 출혈성 방광염, 중추 신경계감염 등 다양한 질환을 일으키는 것으로 알려져있다[1-3]. 사람아데노바이러스는 family Adenoviridae와 genus Mastadenovirus 에 속하는 바이러스로서 70-100 nm 직경의 protein capsid 구조를 가진 nonenveloped DNA virus이다. 아데노바이러스의 capsid는 240 hexon과 12 penton으로 구성되어져 있으며 hexon은 모든 사람아데노바이러스에 공통된 항원부위(antigenic site)와 형특이부위(type specific site)를 가진다. 아데노바이러스는 capsid protein의 variants에 따라 A부터 G까지 7개의 subgenous로 분류할 수 있으며, 현재 70여개의 유전형이 알려져있다[4-6]. 아데노바이러스의 유전형에 따라 일으키는 주된 질환이 달라지게 된다. 유행성 각결막염의 경우 4형, 8형, 19형, 37형이 많고 출혈성 방광염의 경우 11형, 21형, 급성 호흡기 질환 및 폐렴의 경우 3형, 4형, 7형, 21형이 주로 일으키며 유전형에 따라 환자의 증상이나 전염력 그리고 예후도 다르게 나타날 수 있어 유전형의 분석은 중요한 부분으로 알려져있다[7-9]. 그러나 지금까지 우리나라에서 각결막염을 일으키는 아데노바이러스의 혈청형 및 유전형에 따른 각결막염의 임상 양상의 차이를 비교한 보고는 이루어지지 않았다. 이에 저자들은 한국의 유행성 각결막염을 일으키는 아데노바이러스의 주된 유전형을 분석하고 그에 따른 각결막염의 임상 양상을 비교해보고자 하였다.

대상과 방법

2016년 10월 1일부터 2018년 3월 31일까지 18개월간, 본원 안과를 방문한 환자들 중 임상적으로 전형적인 각결막염 증상(결막충혈, 분비물, 결막여포)을 보이고 중합효소연쇄반응(polymerase chain reaction)에서 아데노바이러스가 검출된 환자 17명을 대상으로 아데노바이러스의 유전형을 분석하고 의무기록을 후향적으로 검토하여 당시 세극등현미경검사 소견에 대하여 조사하였다. 본 연구는 연구윤리심의위원회(Institutional Review Board)의 승인을 받았으며 (승인 번호: 2018-10-003), 헬싱키 선언을 준수하였다.

중합효소연쇄반응 검체로부터 아데노바이러스의 DNA의 검출은 QIAamp Viral mini kit (QIAGEN GmbH, Hilden, Germany)를 사용하여 제조사의 방법에 따라 직접 DNA를 추출하였으며 QIAcube platform (Qiagen)으로 분석을 진행하였다. 아데노바이러스 capsid의 헥손 유전자의 염기서열을 분석하기 위해 이에 특이적으로 결합하는 것으로 이미 알려져 있는 primer set (ADHEX1F/AD2)를 사용하여 중합효소연쇄반응(polymerase chain reaction)과 염기서열분석을 실시하였다[10,11]. 이 primer set에 증폭되지 않는 DNA 추출물들에 대해서는 다른 primer set (AD1/AD2)가 PCR에 사용되었다(Table 1). 중합효소연쇄반응(polymerase chain reaction) 산물은 agarose gel에 전기영동하여 밴드를 확인하였으며 DNA서열을 분석하였다. 염기서열분석은 ABI Prism BigDye Terminator version 3.1 (Applied Biosystems, Fosteer City, CA, USA)를 이용하였으며 분석된 염기서열은 National Center for Biotechnology Information (NCBI; http://www.ncbi.nlm.nih.gov)에서 제공하는 Blast를 이용하여 유전자형을 분석하였다. 이 연구에서 얻어진 아데노바이러스의 염기서열과 NCBI에서 제공하는 BLAST를 이용하여 얻은 가장 유사한 서열과의 유전적 관계를 분석하여 계통 분석을 실시하였다. Phylogenic tree는 MEGA program version 7.0 (Kumar, Tamura, Nei 2004; http://www.megasoftware.net)를 이용하여 구성하였다.

결 과

총 20예의 검체 중 17예(85%)에서 아데노바이러스가 분리되었으며 3예의 검체는 본원에서 보유한 primer set로 증폭되지 않았다. 8형이 11예(64.7%), 37형이 5예(29.4%), 64형이 1예였다(Table 2). 아데노바이러스가 검출된 환자들의 연령은 18개월부터 64세(평균 연령 37세)까지 다양하였다. 17예의 검체에 대한 연령별 분포를 아데노바이러스의 유전형별로 살펴보면 8형의 경우 0-10세의 환자가 5건, 31-40세의 환자가 2건, 41-50세의 환자가 3건, 61세 이상의 환자가 1건이었다. 37형의 경우 31-40세의 환자가 4건, 41-50세의 환자가 1건이었다. 64형의 경우 41-50세의 환자가 1건이었다. 환자들의 진단일을 아데노바이러스의 유전형별로 살펴보면 8형의 경우 11월에 6건, 5월에 1건, 8월에 1건, 12월에 1건, 3월에 1건이었으며 11월에 가장 발병률이 높았다. 37형의 경우는 10월에 2건, 11월에 1건, 5월에 1건, 8월에 1건이었으며 64형의 경우는 11월에 1건 발생하였다(Table 3). 아데노바이러스에 의한 각결막염의 임상 양상은 여포, 결막부종, 결막하출혈, 가성막, 결막하 반흔, 점상각막염, 상피결손, 실모양 각막염, 상피밑 침윤을 보였다. 유전형이 확인된 환자들 15명 모두(100%) 여포와 충혈이 확인되었으며 11명(84.6%)은 가성막형성을 보였다(Table 4). 8형의 경우 각막과 결막 모두에서 다양한 임상 증상을 보였지만 특히 각막을 침범한 경우 상피하 침윤이 나타나는 경우가 가장 많았다(Fig. 1). 37형의 경우 결막 증상은 다양하였지만 결막부종은 일으키지 않았으며, 각막을 침범하는 경우 모두 실모양각막염을 보였다(Fig. 2). 분리된 아데노바이러스의 공통 항원부위이자 특이적 항원인 헥손유전자의 계통분류학적 분석에서는 subgroup D에 속하는 17예 모두 같은 혈청형인 경우 동일한 염기서열을 갖고 있었으며, 기존의 보고된 아데노바이러스 8형, 37형, 64형과 일치하여 유전자 변이는 없는 것을 확인하였다(Fig. 3).

고 찰

아데노바이러스에 의한 각결막염은 바이러스성 결막염의 65-90%를 차지하고 있는 안과에서 매우 흔한 질환이고 발병률과 전염성이 높아 대규모 발생도 흔한 편이다. 아데노바이러스는 55개 이상의 혈청형이 알려져 있고, 이 중 유행성 각결막염을 일으키는 아데노바이러스는 결막상피에 높은 친화력을 보이는 subgenous D에 속하는 4형, 8형, 19형, 37형으로 알려져있다[10-13]. 아데노바이러스의 유전형 분석에 결막도찰 검체를 통한 아데노바이러스 헥손 유전자의 중합효소연쇄반응(polymerase chain reaction)과 direct sequencing은 아데노바이러스의 검출부터 혈청형을 단시간에 분석할 수 있는 쉽고 빠르며 오차가 적은 유용한 방법이다[14-16]. 이 방법을 이용하여 확인한 해외의 유행성 각결막염을 일으키는 주된 혈청형은 이미 보고되어 있지만 아직까지 우리나라에서 유행성 각결막염을 일으키는 아데노바이러스의 유전형이 대한 연구가 많지 않아 본 연구에서는 2016년 10월 1일부터 2018년 3월 31일까지 본원을 방문한 각결막염 환자들을 대상으로 한국에서 발생하는 아데노바이러스의 주된 유전형을 확인하고자 하였다. 또한 확인된 유전형별 발생 연령, 발생 시기 및 임상 증상의 차이를 비교해보고자 하였다. 연구 시기 동안, 한국에서 아데노바이러스에 의한 각결막염을 일으키는 바이러스는 8형, 37형이었으며 이는 기존의 보고와 일치하는 결과였다. Tsai et al [17]이 생쥐를 이용한 실험에서도 같은 결과를 얻었으며 이는 Aoki and Tagawa [18]가 Sapporo에서의 10년간의 연구에서도 8형, 19형, 37형의 순으로 전신 증상 없이 심한 결막염을 일으킨다는 것을 동일하게 확인하였다. Jin et al [19]이 Vietnam, Hanoi에서 시행한 연구에서도 아데노바이러스 각결막염의 역학연구에서 8형이 가장 높은 비율로 발생한다고 하였다. 본 연구에서는 발생 연령의 경우 전 유전형에서 모두 31-50세에서 가장 발생률이 높았으며 특히 8형의 경우 특징적으로 0-10세 사이의 소아에서 발생하였다. 아데노바이러스 각결막염은 전염성이 매우 높으므로 사람 간 전파가 쉬운 다수의 군중이 모이게 되는 곳에서 흔히 전파가 되는 만큼 가장 활발한 활동을 하는 연령대에서 발생률이 높게 나온것으로 생각된다. 유사한 이유로, 8형에서 소아의 비율이 높은 것은 8형이 가장 많은 비율을 차지하면서 소아의 경우 유치원, 학교라는 집단생활을 통해 직접 접촉으로 신속히 전파되어 더 발생률이 높게 나온 것으로 결론을 낼 수가 있다.

이 연구에서 결막충혈, 여포형성과 거짓막형성이 아데노바이러스 8형, 37형감염시 주된 임상 증상이었다. 8형의 경우 결막 및 각막에 다양한 임상 증상을 보였고 특히 각막증상 중 상피하 침윤이 가장 많았고 8형에서 상피하 침윤이 많다는 이전의 연구 보고와 동일하였다[20]. 이에 비하여 37형의 경우 주로 결막에 경한 증상을 보였지만 특이하게 결막부종이 나타나지는 않았다. 적은 수로 각막을 침범하는 경우는 실모양각막염의 형태만 보였다(Table 4). 같은 subgenous에 속하는 아데노바이러스는 특성이 유사하여 임상 양상이 유사할 것으로 예상되었지만, 이번 연구의 결과에서 아데노바이러스의 subgenous가 같더라도 유전형에 따라 나타나는 임상 증상이 달랐다. 물론 본 연구에서 대상수가 소수라는 제한점이 있지만, 본 연구의 결과는 아데노바이러스의 유전형별 임상 양상의 차이가 있을 가능성을 시사한다. 추후 좀 더 오랜 기간에 걸쳐 다수의 기관에서 많은 검체를 얻어 추가적인 연구가 진행이 된다면 우리나라에서 발생하는 아데노바이러스에 의한 각결막염의 주된 유전형 및 다양한 유전형별 임상 양상의 차이를 명확히 나타낼 수 있을 것이다. 이를 통해 초기 비특이적인 증상을 보이는 경우 중합효소연쇄반응(polymerase chain reaction) 및 염기서열분석을 통한 아데노바이러스의 유전형의 확인하여 각결막염의 조기 발견 및 발생률 감소, 유전형에 따른 추가 임상 증상의 발생 예측 및 예후관리에 도움을 줄 수 있는 기초 역학자료에 활용될 수 있을 것으로 생각된다.
