A Case of Capsular Block Syndrome after Diffractive Multifocal Intraocular Lens Implantation

Abstract
Purpose
We report ultrasound biomicroscopic (UBM) feature and surgical treatment of early onset capsular block syndrome after diffractive multifocal intraocular lens (IOL) implantation. Methods: A 66-year-old woman underwent uncomplicated phacoemulsification and Acri.Lisa 366D diffractive multifocal IOL (Carl Zeiss Meditec, Jena, Germany) of plate-type one-piece design was implanted. Results: Manifest refraction spherical equivalent (MRSE) was -2.75 diopter (D) the next day and -2.00 D at one week. Slitlamp examination revealed a gap between posterior capsule and IOL, anterior shift of IOL and full coverage of optic with continuous curvilinear capsulorrhexis margin. Ultrasound biomicroscopy showed anterior bowing of IOL and touch of optic to iris margin but IOP was in normal range. At 8 days postoperatively, viscoelastics behind IOL was removed by bimanual irrigation and aspiration after mydriasis. Myopia decreased rapidly after second operation. MRSE was -0.50 D till one week and plano at one month. Uncorrected distant vision was 1.0 and near vision was J1. (Korean J Optom Vis Sci 2011;10(1):51-55)
