Retinal Hemorrhage after Screening Examination for Retinopathy of Prematurity in a Premature Infant

Abstract
Purpose
To report retinal hemorrhage in a premature infant by screening examination for retinopathy of prematurity (ROP). Methods: We reviewed medical record retrospectively. Results: An 1,190 g 30 week 5 day gestation infant was consulted for screening examination about ROP. First fundus examination conducted at 33 week 3 day gestation age. There was no retinal hemorrhage, plus sign, demarcation line or ridge and shows Zone II vascularization. After 2 weeks later second fundus examination, multiple round shape of retinal hemorrhage developed in left eye. And the very next day, retinal hemorrhage newly developed same in which left eye was observed in right eye. Retinal hemorrhage spontaneously resolved completely after 2 weeks later. There was no sign of ROP on fundus examination of follow up period. Conclusions: This case shows how susceptible to rupture the infant retinal vascular system is. There is possibility of microtrauma and compressive effect through sclera indentation or Valsalva effect causes these hemorrhages of premature infant. But exact pathogenesis of these hemorrhages is unclear. (Korean J Optom Vis Sci 2009;8(2):78-80)
