| A Case of Facial and Periorbital Cellulitis with Orbital Emphysema after Orbital Wall Reconstruction |

| Abstract |
Purpose To report a case of facial and periorbital cellulitis with orbital emphysema after orbital wall reconstruction using alloplastic implant. Methods: A 25-year-old woman visited our clinic due to right periorbital swelling after traffic accident. A fracture of the right medial wall fracture was identified in an orbital CT scan. Orbital wall reconstruction was performed with MedporⰒ sheet implantation over the orbital wall defect. On the first day after surgery, diplopia, right exodeviation and limitation of the infraduction and adduction of the right eye was shown. Intraorbital emphysema adjacent to the implanted MedporⰒ and the swollen right medial rectus were identified in the orbital CT scan. Results: At the 22th days after surgery, erythematous swelling and tenderness were developed around the right periorbital area extending to the ipsilateral cheek area. MedporⰒ sheet was removed and systemic antibiotic treatment was performed. There was no growth of any specific organisms but relatively increased inflammatory cells in tissue biopsy. Facial swelling was recovered after MedporⰒ sheet removal. Conclusions: We should consider periorbital cellulitis in case of postoperative inflammation and limitation of ductions. Intraorbital emphysema can be induce inflammatory reactions as a kind of dead space, so that surgeons need to be careful with the orbital implantation. (Korean J Optom Vis Sci 2009;8(1):33-37) |
