Leakage into Anterior Chamber of Triamcinolone and Intraocular Pressure Increase after Intravitreal Triamcinolone Injection

Abstract
Purpose
To report a case of intraocular pressure increase after intravitreal triamcinoline injection because of influx of triamcinolone into anterior chamber. Case Summary: Forty-five year old man visitied hospital with a complain of abrupt visual field loss. Retinal detachment with a tear in superior nasal part of posterior pole. Vitrectomy with a C3F8 gas injection was performed. At 4 post-operative months visual acuity decrease and cystoid macular edema was found, so intravitreal triamcinolone injection was performed. One day after injection, triamcinolone precipitate was found in the anterior chamber and then intraocular pressure (IOP) was increased to the 42 mmHg at 7th day after injection. IOP was normalized after IOP lowering drug use. Because cystoids macular edema was not improved until 2 months after injection, Additional posterior subtenon triamcinolone injection was performed. Cystoids macular edema was resolved and IOP increase was not appeared. Conclusions: In case of intravitreal triamcinolone injection of vitrectomised eye, Influx of triamcinolone into anterior chamber and Intraocular pressure increase may occur especially when patient has a prone position sleep pattern. So, additional teaching was required that patient must not have a prone position in sleep. (Korean J Optom Vis Sci 2012;11(1):113-116)
