의인성 안구건조증에서 신경펩티드의 변화와 가능한 역할

Abstract

최근 안구건조증의 발병기전에 각막신경의 역할이 부각되고 있다. 신경펩티드가 신경계와 면역계의 중요한 매개체로 알려져 있다는 점을 고려하면, 신경 말단에서 분비되는 신경펩티드가 유발하고 조절하는 신경성염증은 안구건조증의 발병기전에도 관여할 것으로 기대된다. 최근에 눈 수술 및 콘택트렌즈 사용 후에 발생하는 안구건조증에 대한 관심이 증가하면서, 의인성 안구건조증은 안구건조증의 중요한 분야로 각광을 받고 있다. 이 논문에서는 안구건조증의 발병기전과 관련하여 신경펩티드의 역할에 대한 최근 연구 결과들을 간단히 살펴보고, 특히 의인성 안구건조증에서 신경펩티드의 변화 및 가능한 역할에 대한 기존 연구 결과들을 정리하고자 한다.

Abstract

Recently, the role of the corneal nerve in the pathogenesis of dry eye disease (DED) has been highlighted. Considering that neuropeptides are known as important mediators between the nervous and immune systems, neurogenic inflammation induced and regulated by neuropeptides secreted from nerve terminals is expected to be involved in the pathogenesis of DED. And iatrogenic DED has been recently spotlighted as an important field with increasing interest in DED that occurs after ocular surgery or contact lens use. In this paper, recent studies on the role of neuropeptides in the pathogenesis of DED, in particular, the changes and possible roles of neuropeptides in iatrogenic DED were reviewed.

우리 몸은 기계적 손상이나 병원체의 침입에 대항하기 위한 시스템을 갖추고 있는데, 면역계와 신경계가 상호작용을 통하여 이러한 역할을 맡게 된다[1]. 이 과정에서 면역계와 신경계 상호작용에 중요한 역할을 하는 신경펩티드(neuropeptide), 이를 통하여 유발되고 조절되는 신경성염증(neurogenic inflammation)이 중요한 역할을 한다[2-5]. 하지만 이러한 신경성염증은 일부 질환에서는 질환을 유발하는 기전으로 작용할 수도 있음이 보고되고 있고[5-10], 안구건조증의 발병기전에도 관여할 가능성도 오래 전부터 제시되었다[2,11-13]. 최근에 눈 수술 및 콘택트렌즈 사용 후에 발생하는 안구건조증에 대한 관심이 증가하면서, 의인성 안구건조증은 Tear Film & Ocular Surface Society의 국제안구건조증워크숍 II (International Dry Eye Workshop II)에서 독립된 주제로 다루어질 정도로 각광을 받고 있는 분야이다[14]. 이 논문에서는 안구건조증의 발병기전과 관련하여 신경펩티드의 역할에 대한 최근 연구 결과들과 특히 의인성 안구건조증에서 신경펩티드의 변화를 본 논문들을 고찰하여 향후 이 분야 연구의 방향을 제시하고자 한다.

본 론

신경성염증의 양면성

우리 몸은 외부에서 들어오는 기계적 손상이나 병원체의 침입에 대항하기 위한 시스템을 갖추고 있으며 눈도 예외는 아니다[1]. 면역 관용이 잘 알려진 전방과는 달리, 눈 표면에서도 이러한 손상과 병원체에 대한 끊임없는 감시가 이루어지고 있으며, 이 과정에서 신경성염증이 중요한 역할을 하고 있다[2-4]. 기계적 손상에 의한 신경말단에 대한 자극이나 손상은 즉각적인 신경펩티드의 유리를 일으키게 되고 이는 염증의 유발과 조절을 일으키게 되어 신경성염증이라고 불리는 일련의 과정이 일어나게 된다[2,3]. 신경성염증은 느린 전도속도를 가지고 있는 가는 신경섬유(unmyelinated C-fibers와 finely myelinated A-fibers, Aδ fibers)에 의하여 주로 유발되는데, 이러한 신경들은 눈 표면에 치밀하게 분포하고 있다[2,15]. 이렇게 분비되는 작은 분자의 펩티드인 신경펩티드는 신경계와 면역계의 상호작용에서 중요한 역할을 담당하게 된다[2,5]. 신경성염증은 상처 회복과 감염으로부터의 보호를 위하여 유발이 되고 실제로 중요한 역할을 하지만[1,16], 그 부작용으로 알레르기병, 아토피피부염, 건선(psoriasis), 주사(rosacea), 섬유근통(fibromyalgia), 염증성장 질환(inflammatory bowel diseases), 자가면역장염(autoimmune enteritis), 외사성뇌손상(traumatic brain injury), 통증, 편두통 등 일부 질환에서는 발병기전으로도 작용할 수 있음이 보고되고 있다[5-10].

대표적인 신경펩티드와 그 수용체들

외부의 자극에 반응하여 발생하는 면역 반응의 유발과 조절에 관여하는 것으로 알려진 대표적인 신경펩티드로는 substance P (SP), calcitonin gene-related peptide (CGRP), vasoactive intestinal peptide (VIP), neuropeptide Y (NPY)를 들 수 있다[17-22].
SP는 11개의 아미노산으로 이루어진 tachykinin family의 신경펩티드로 주로 신경세포와 염증세포에서 분비된다[23,24]. SP는 neurokinin-1 receptor (NK1R), NK2R, NK3R의 3가지 tachykinin 수용체에 결합하여 작용하게 되는데[25], 특히 NK1R에 높은 친화도로 결합하여 여러 가지 생리적, 병리적 활동을 일으키게 된다[26-28]. CGRP는 37개의 아미노산으로 이루어진 다기능 신경펩티드로, 수용체로는 calcitonin receptor-like receptor (CLR), receptor activity-modifying protein 1 (RAMP1), receptor component protein (RCP)이 알려져 있다[29,30]. 전통적으로 CGRP 수용체는 CGRP1과 CGRP2로 나뉘고 서로 다른 민감도를 보이는데, 조직에 따라서 친화도가 다르기 때문에 다양한 반응을 보이는 것으로 알려져 있다[31]. VIP는 전구물질로부터 28개의 아미노산으로 이루어진 활성형으로 합성되며[32,33], 주로 중추신경계에서 유래한 부교감신경 말단에 발현된다. VIP와 pituitary adenylate cyclase-activating polypeptide (PACAP)는 두 가지의 연관성이 높은 신경펩티드로, VIP/PACAP receptor 1 (VPAC1R), VPAC2R, PACAP receptor 1 (PAC1R) 세 가지 수용체에 서로 다른 친화도로 결합한다[30,34,35]. NPY는 36개의 아미노산으로 이루어졌으며 교감신경 말단에 주로 분포한다[36-38]. NPY수용체는 “Y”수용체(Y1, Y2, Y3, Y4, Y5, Y6)로 불리고, 포유동물에서는 5가지(Y1, Y2, Y4, Y5, Y6), 사람에서는 4가지(hY1, hY2, hY4, hY5)만 기능하는 것으로 보고되고 있다[30,39].

눈 표면에서 신경펩티드의 발현

눈 표면은 각막, 결막, 눈물샘, 마이봄샘, 눈물로 구성되는 기능적 단위로 삼차신경의 안분지에서 기원한 감각신경, 위목신경절(superior cervical ganglion)에서 기원한 교감신경, 날개입천장신경절(pterygopalatine ganglion)에서 기원한 부교감신경이 분포된다(Fig. 1) [40-42]. SP, CGRP, NPY, VIP와 같은 신경펩티드와 그 수용체도 눈 표면의 신경섬유와 면역세포에 분포하는데[3,43,44], 이는 저자들의 이전 논문에 잘 정리되어 있다[45].

안구건조증의 발병기전에서 신경펩티드의 역할

최근에 동물실험을 이용한 몇몇 연구를 통하여 안구건조증의 발병기전에서 신경펩티드와 그 수용체의 역할이 보고되고 있다. 건조 스트레스를 이용한 건성안유도챔버(controlled-environment chamber) 안구건조증 동물모델에서 각막, 결막, 삼차신경절에 SP가 증가함과 SP의 주요 수용체인 NK1R을 길항함으로써 건조 스트레스에 의하여 유도된 안구건조증의 임상 소견이 완화됨이 보고되었다[46-49]. 건조 스트레스에 의하여 증가된 SP가 미성숙 상태의 항원제시세포(antigen presenting cell)를 성숙시키고, 안구건조증 발병에 중요한 작동세포로 최근에 각광을 받고 있는 type 17 helper T (Th17)세포로의 분화를 유도하는 것과 이러한 변화가 NK1R을 길항함으로써 억제될 수 있음이 Yu et al [46]에 의하여 보고되었다. 또한, 배수림프절(draining lymph node)에서 Th17세포의 유도를 억제하는 것으로 알려진 조절T세포(regulatory T cell, Treg)의 빈도와 기능이 SP에 의하여 감소하고, 마찬가지로 NK1R을 길항함으로써 Treg의 조절기능을 복원시킬 수 있음이 보고되었다[48]. 안구건조증 면역학적 발병기전에서 중요한 역할을 할 것으로 생각되는 각막 림프관 신생에도 SP/NK1R system이 관여함이 증명되었고, 특히 림프관세포에서 vascular endothelial growth factor receptor 3 (VEGFR3)의 발현 증가를 통하여 SP가 림프관 신생에 관여함이 림프관세포배양을 통하여 증명되었다[49]. 하지만 SP가 삼투압 스트레스에 의한 각막상피세포의 세포자멸사(apoptosis)에 대한 보호 효과를 갖고[50], 직접적으로 조직재생을 돕는 M2형의 큰포식세포(macrophages)를 유도한다는 보고도 있어[51], 안구건조증, 그에 따른 조직손상의 단계에 따라서 SP의 역할이 다를 가능성을 고려해 볼 필요가 있고 추가적인 연구가 필요하다.
안구건조증 환자의 눈 표면에서 염증성 시토카인(proinflammatory cytokines)의 농도 증가가 보고되고, 쇼그렌증후군 환자의 침에서 뉴로펩티드 농도의 변화가 관찰되면서, 신경성염증이 안구건조증의 발병에도 중요한 역할을 할 것으로 기대가 되었으나 실제로 안구건조증 환자의 눈물에서 뉴로펩티드의 변화를 보고한 연구는 매우 제한적이다[52-56]. Lambiase et al [57]은 19명의 안구건조증 환자(쇼그렌 5명, 비쇼그렌 10명, 안천포창 4명)와 대조군 12명의 눈물에서 뉴로펩티드 농도를 분석하여 안구건조증 환자에서 CGRP와 NPY 농도가 감소하고 신경성장인자(nerve growth factor)의 농도가 증가되었음을 보고하였다. 이 연구에서는 SP와 VIP 농도는 변하지 않았는데, 특히 SP 농도는 쇼그렌, 비쇼그렌 모두에서 증가하지 않아 기존의 동물실험 결과들과 다른 결과를 보여주었다. 이에 대한 해석은 주의가 필요한데, 염증성 시토카인이면서 안구건조증에서 증가하는 것으로 알려진 interleukin-6의 눈물내 농도 증가가 각막 기저신경얼기(subbasal nerve plexus)의 신경섬유 두께를 감소시켰다는 보고하였고[58,59], 각막 감각저하(corneal hypesthesia) 또는 당뇨성 말초신경병증(diabetic peripheral neuropathy) 상태에서 눈물의 SP가 감소하였다는 보고를 종합적으로 고려할 때[60-62], 안구건조증이 진행되면서 발생한 각막신경의 감소가 증가된 SP 분비를 상쇄시켰을 가능성을 고려해볼 수 있고 이에 대해서는 추가 연구가 필요하다.

의인성 안구건조증과 신경펩티드

앞서 살펴본 바와 같이 동물실험을 통한 안구건조증의 발병기전에 대한 연구에서와 달리 실제 안구건조증 환자를 대상으로 한 임상 연구에서 SP의 변화와 역할은 아직 밝혀지지 않았다. 하지만 의인성 안구건조증과 관련하여서는 SP가 임상적으로 의미 있는 변화를 보이고, 발병기전과 관련이 있을 가능성을 유추할 수 있는 결과들이 좀 더 발견된다.
굴절수술 후에는 눈물 생성, 눈물의 질, 눈깜빡임 반사의 감소로 인하여 안구건조증이 발생하는 것이 알려져 있고[63], 라식(laser-assisted in situ keratomileusis, LASIK)이나 굴절교정레이저각막절제술(photorefractive keratectomy, PRK) 후에 눈물 내 신경펩티드의 농도가 변화됨이 보고되고 있다[63-67]. 라식수술 후 눈물의 SP 농도는 3개월까지 증가됨이 보고되었지만[65,66], 12개월째에는 대조군과 차이가 없다고 보고되었다[67]. 라식수술 후에는 눈물의 SP 농도가 각막신경섬유 밀도와 반비례해서, 신경밀도가 높을 때 눈물 내 SP 농도가 낮았다고 보고되었다[65]. 라식수술 후 눈물의 CGRP 농도에 대한 결과는 SP와는 반대로 전향적인 추적 연구에서는 3개월까지 변화가 없었다고 보고되고 있고[66], 수술 후 12개월째에 시행한 단면 연구(cross-sectional study)에서는 정상대조군에 비해서 유의하게 높았다고 보고되었다[67]. PRK 수술 전과 비교한 전향적인 연구에서 눈물의 CGRP 농도는 PRK 후 2일까지 증가하여 최대치에 도달한 후 7일째까지 감소함이 보고되었다[64]. 처음 이틀간 증가한 CGRP는 손상된 각막신경으로부터 유리된 것으로 생각되는데, 수술 직후의 눈물 분비 증가에도 불구하고 농도가 감소하지 않아서 상당량의 CGRP가 손상된 신경으로부터 유리되었음을 유추할 수 있다[64]. 정리하자면, 굴절수술 후에는 눈물의 SP와 CGRP 농도의 변화가 보고되고 있으며, 이는 굴절수술에 의한 각막신경손상과 연관될 가능성을 생각할 수 있다. 하지만 변화된 신경펩티드가 술 후 안구건조증의 발생에 기여하는 부분은 향후 좀 더 연구가 필요하다.
백내장수술 후 안구건조증 악화에 대해서는 최근에 임상적으로 관심이 많은 부분이다. 하지만 아직까지 백내장수술 후 안구건조증 발생과 관련하여 신경펩티드의 역할을 보는 연구는 저자들의 검색에서는 발견되지 않았다. 다만 술 후 안구건조증은 아니지만 수술 시 통증과 관련하여, 당뇨 환자에서 연이어 양안 백내장수술을 시행한(1일 또는 1주 간격) 경우 안구방수의 SP와 monocyte chemoattractant protein 1이 나중에 수술한 눈에서 유의하게 높아서, 교감성 염증 반응(sympathetic inflammatory responses)을 통하여 나중에 수술한 눈에서 더 심한 통증을 유발할 가능성을 보고한 논문이 최근에 발표되었다[68].
콘택트렌즈 착용은 눈물층의 불안정, 눈물 증발의 증가, 눈물막 반달띠(tear film meniscus)의 저하, 기본눈물(basal tear) 회전율 저하, 눈물 삼투압 증가, 기름층 변화와 렌즈 습윤성 저하, 결막 충혈과 상피손상 등을 초래하여 눈물막과 눈 표면의 생리적 변화를 초래하는 것으로 알려져 있다[14,69]. 하지만 콘택트렌즈 착용에 따른 눈물의 신경펩티드 변화에 대한 연구는 아직까지 여지가 많다. 콘택트렌즈 착용자 20명과 비착용자 20명을 비교한 연구에서는 SP와 CGRP의 농도는 유의한 차이가 없었고, 함께 분석한 각막신경 형태와 각막지각도 차이가 없었다[70]. 하지만 신경의 밀도와 눈물의 CGRP 농도 및 각막지각은 유의한 상관관계가 관찰되었다. 반면, 콘택트렌즈 착용자 중 불편감을 느끼는 30명과 불편감을 느끼지 않는 30명을 비교하였을 때는 눈물의 SP가 불편감을 느끼는 군에서 유의하게 높았다[69]. 아직 연구 결과가 많지 않아서 확정적으로 단정할 수는 없지만, 눈물 중 신경펩티드의 농도, 특히 SP는 콘택트렌즈 착용 자체보다는 콘택트렌즈 불편감과 더 관련이 있을 가능성이 있어 콘택트렌즈 불편감 또는 콘택트렌즈로 인한 안구건조증과의 관련 가능성을 시사한다고 하겠다. 눈물의 CGRP 농도 역시 콘택트렌즈 착용 자체보다는 각막신경의 밀도와 좀 더 관련된 것으로 보인다.

결 론

이 논문에서는 안구건조증의 발병기전과 관련하여 신경펩티드에 대한 최근 실험적 연구 결과들과 현재까지의 임상 연구 결과들 그리고 의인성 안구건조증과 관련하여 신경펩티드의 변화를 본 임상 연구들을 간단히 살펴보았다. 아직까지 안구건조증의 발병기전과 관련하여 신경펩티드의 역할은 명확하지 않지만, 기전을 고려한 가능성과 최근의 연구 결과들을 종합해보면 신경펩티드가 안구건조증 발병에 관여할 가능성은 높을 것으로 생각된다. 하지만 실험적 연구가 실제 임상 연구 결과와 차이를 보이는 부분이 가장 큰 문제점인데, 이는 안구건조증의 발병 시기와 중증도에 따라서 신경펩티드의 역할이 상반될 수 있고, 안구건조증 진행에 따른 각막신경손상이 증가된 신경펩티드의 분비를 가릴 수 있는데[71-73], 기존 임상 연구는 그러한 부분이 고려되지 못한 것이 한계점으로 생각된다. 그런 점에서 비교적 발병 시기가 명확하고 손상의 정도가 일정한 의인성 안구건조증은 임상 연구를 위한 좋은 안구건조증 모델로 기대된다. 아직은 빈약하지만 현재까지의 연구 결과들이 어느 정도 의인성 안구건조증의 발병에 신경펩티드가 관여할 가능성을 보여주고 있다고 생각되고, 관련된 연구 결과들이 좀 더 쌓여가면 안구건조증의 발병기전에서 신경펩티드의 역할이 좀 더 명확해질 수 있을 것으로 기대한다.
