약시 환아의 성공적인 가림치료에서 중간 성취 기간의 임상적 의미

Abstract

Purpose

To evaluate the factors affecting total occlusion duration and to determine whether half success duration (the time to reach an intermediate visual acuity between the initial visual acuity and 1.0) affects the total duration of treatment in amblyopia patients when the best-corrected visual acuity (BCVA) improves to that of the fellow eye after occlusion.

Methods

Fifty-three pediatric patients with amblyopia who achieved the same BCVA as that of the fellow eye after occlusion were selected. The total duration and half success duration were compared according to sex, initial visual acuity, age, spherical equivalent, cause of amblyopia (anisometropia, strabismus, or combined), as well as other factors.

Results

The mean age of the initial amblyopic patients was 4.6 years, and the mean duration of total occlusion treatment was 23.15 months. The mean half success duration was 9.98 months. The duration of total occlusion treatment was longer in children whose spherical equivalent of the amblyopic eye was over +3.0 diopters. The half success duration was longer in amblyopic patients with esotropia whose angle of deviation exceeded 20 prism diopters. However, there were no significant differences according to other factors. The total occlusion duration was shorter in patients whose half success duration was less than 12 months.

Conclusions

The treatment duration that was needed to achieve a BCVA close to that of the fellow eye was shortened when the spherical equivalent of the amblyopic eye was below +3.0 diopters. The half success duration was shortened when the angle of deviation was well below 20 prism diopters in patients with esotropia. Additionally, patients with a short half success duration had a shorter total occlusion duration. These factors can be used to predict the duration of occlusion treatment.

시력은 생후 2-3개월에 급격히 발달하고 6-7세경에 완성되며, 이 시기에 유발요인에 의해서 약시가 발생할 수 있다. 약시의 유병률은 일반적으로 2-4% 정도로 알려져 있으며[1-4] 약시치료는 시력의 정상적인 신경발달을 막는 방해신호를 차단하는 것에 있으며, 이를 위해 아트로핀 처벌치료, 가림치료, 안경치료 등의 치료가 알려져 있다. 약시치료의 성공률은 문헌마다 다르지만 Pediatric Eye Disease Investigative group (PEDIG)에 따르면, 7세 이하의 중등도약시를 가진 환아들에 대하여 약 75%의 성공률을 보이고 있다[1].
그동안 약시의 치료 효과에 영향을 미치는 인자들에 대한 여러 연구를 통해, 치료 시작 시기가 늦은 경우, 초진시력이 나쁜 경우, 난시가 있는 경우 그 결과가 나쁘다고 알려져 있다[5-9]. 치료 기간에 영향을 주는 인자로는 치료 시작 나이, 초진 시력, 구면렌즈대응치 등이 있다는 연구가 있으나 아직 부족한 실정이다[10]. 이에 저자들은 약시치료 후 1.0이 되어 성공적으로 치료된 환자들의 임상양상을 분석하여 총 가림치료 기간과 초진시력에서 1.0 사이의 중간시력에 도달하는 데 걸리는 기간인 중간 성취 기간(half success duration)에 영향을 미치는 인자에 대해 알아보고자 하였다. 또한 치료 초기의 시력호전 속도가 총 치료 기간에 미치는 영향 또한 분석해 보았다.

대상과 방법

2014년 1월부터 2016년 12월까지 본원에 내원하여 단안 약시로 진단받고, 가림치료를 시행받은 환아 가운데 약시안의 최대교정시력이 스넬렌시력표에서 반대안과 동일한 시력에 도달한 환아 53명을 대상으로 후향적 연구를 시행하였다. 본 연구는 헬싱키선언을 준수하였고, 본원 연구윤리심의위원회(Institutional Review Board, IRB)의 승인(승인 번호: 2020-467)을 받아 진행하였다. 전신 및 신경계 질환이 있거나, 초진 시 나이가 어려서 스넬렌시력표로 시력 측정이 힘든 환아는 대상에서 제외하였다. 모든 환아를 대상으로 세극등현미경검사, 안구운동검사, 교대프리즘가림검사, 조절마비 굴절검사를 시행하였다.
성별, 가림치료 시작 나이, 약시안 시력, 약시 원인, 사시 종류 및 사시각, 최대교정시력이 반대안과 동일한 수치까지 도달하는 기간을 조사하였다. 가림치료 시작 시의 시력과 반대안과 동일한 시력 사이의 중간값에 도달하는 데 걸리는 기간도 함께 조사하였으며 이를 중간 성취 기간(half success)이라고 정의하였다.
모든 시력은 스넬렌시력표로 최대교정시력을 측정한 후 logarithm of the minimum angle of resolution (logMAR) 시력으로 환산하여 분석하였다. 약시의 정의는 스넬렌시력표를 기준으로 하여 두 눈의 최대교정시력이 두 줄 이상 차이가 나는 경우로 하였다. 약시의 분류는 사시약시, 굴절부등약시, 두 종류가 혼합된 복합약시로 하였다. 약시 진단시에는 조절마비 굴절검사를 시행하였으며, 근시 또는 원시가 2.0 diopters (D), 난시는 1.5 D 이상 차이 나는 경우를 굴절부등약시로 정의하였다. 사시약시는 원거리 및 근거리에서 시행한 프리즘 교대가림검사상 10 prism diopters (PD) 이상의 현성사시가 있는 경우로 하였고, 굴절부등을 보이면서 사시약시의 조건에 해당하는 경우를 혼합약시로 분류하였다. 약시안의 구면렌즈대응치(spherical equivalent)를 구하여 0 미만, 0 이상 +3.0 D 이하, +3.0 D 초과의 3군으로 분류하였다.
가림치료는 깨어 있는 상태에서, 하루 평균 2-6시간 정도의 눈가림으로 시작하였으며, 추적 관찰을 하며 환자의 상태에 따라 가림치료의 시간을 조정하였다. 안경이 필요한 환아는 안경을 착용한 상태에서 약시치료를 병행하였다.

결 과

가림치료 후 약시안의 시력이 반대안과 동일하게 도달한 환아 53명 중 남자 24명, 여자 29명이었고, 가림치료 시작의 평균연령은 4.6 ± 1.4세였다(Table 1). 성별에 따른 치료 기간을 비교했을 때 남자의 총 가림기간은 평균 25.38개월, 여자는 평균 21.31개월로 두 군 간에 유의한 차이를 보이지 않았고(p=0.106), 중간 성취 기간도 남자 평균 9.08개월, 여자 평균 10.72개월로 차이가 없었다(p=0.893) (Table 2). 가림치료 시작 시 연령이 7세 미만인 경우는 38명, 7세 이상이 15명이었다. 두 군 간에 약시안이 아닌 눈과 약시안의 시력, 두 눈의 시력 차이는 통계적으로 두 군 간에 유의성이 없었으며, 가림치료를 시작한 나이가 7세 이상인 군의 총 가림 기간은 24.87개월, 중간 성취 기간은 9.63개월로 7세 미만인 군의 총 가림 기간 21.28개월, 중간 성취 기간 11.54개월과 유의한 차이는 없었다(p=0.282, p=0.466) (Table 3). 가림치료 시작 시 약시안의 시력(logMAR)이 0.3 이하인 경우는 30명, 0.3보다 큰 경우는 23명이었다. 총 가림기간은 0.3 이하인 군이 21.83개월, 0.3보다 큰 군이 24.87개월로 차이가 없었고(p=0.074), 중간 성취 기간 또한 11.43개월과 8.09개월로 유의한 차이를 보이지 않았다(p=0.387) (Table 4).
약시 원인에 따른 분류는 굴절부등약시 34명, 사시약시 11명, 혼합약시 8명이었다. 사시와 혼합약시 19명 중 외사시는 8명이었으며, 모두 간헐외사시로 사시각은 평균 근거리, 원거리에서 15.50 PD가 관찰되었다. 내사시 환자는 11명이었고, 근거리에서 16.73 PD, 원거리에서 13.27 PD의 평균 사시각을 보였다(Table 1). 사시 유형에 따른 총 가림 기간은 내사시인 군에서 36.45개월, 외사시 군에서 21.00개월로 통계적인 차이가 없었고(p=0.062), 중간 성취 기간 또한 내사시에서 16.00개월, 외사시에서 11.75개월로 유의한 차이가 없었다(p=0.840) (Table 5). 근거리와 원거리에서 사시각이 20 PD 미만인 군과 이상인 군으로 나누어 가림치료 기간을 비교하였을 때, 내사시 환자에서 원거리 사시각이 20 PD 미만인 군은 중간 성취 기간 9.88개월로 20 PD 이상인 군의 32.33개월보다 통계적으로 유의하게 짧았다(p=0.012) (Table 6). 그 외에 근거리 및 원거리에서 사시각에 따른 가림치료 기간의 차이는 없었다(Table 7-9).
약시안의 구면렌즈대응치가 0 D 미만인 경우는 8명, 0 이상 3.0 D 이하인 경우는 24명, +3.0 D보다 큰 경우는 21명이었다. 약시안의 구면렌즈대응치가 +3.0 D 이상인 군의 총 가림기간이 평균 31.95개월로 가장 길었고, 다른 군에 비해 통계적으로 유의한 결과를 보였다. 중간 성취 기간은 세 군 간에 차이가 없었다(Table 10).
중간 성취 기간과 총 가림 기간의 관계를 분석하기 위해 중간 성취 기간이 12개월 미만인 군과 이상인 군으로 나누어 총 가림 기간을 비교하였을 때 중간 성취 기간이 12개월 미만인 군은 평균 18.14개월로 12개월 이상인 군의 총 가림 기간인 평균 32.89개월보다 통계적으로 유의하게 짧았다(p<0.001) (Table 11).

고 찰

약시의 치료에 영향을 미치는 요인은 여러 가지가 연구되어 왔는데, 그 중 치료를 시작한 나이가 많을수록 약시치료에 걸리는 기간이 길고 치료에 반응을 잘 하지 않는다고 알려져 있다[11-13]. 나이가 많은 경우 장기간 약시에 대한 적응이 되어 있어 가림치료에 대한 순응도 및 잠재되어 있는 시피질에 대한 자극이 떨어져 치료의 반응이 좋지 않다[11,13]. 그러나 학동기 아동 또는 9세 이상의 약시에서도 가림치료가 효과가 있다는 보고도 있다[9].
또한 약시안의 나쁜 초기시력은 긴 치료 기간과 치료 실패와 연관이 있었으며, PEDIG의 연구에서는 0.2보다 좋은 시력에서는 가림치료와 아트로핀치료 모두에서 좋은 반응을 보였다는 결과도 있었다[5]. 본 연구에서는 가림치료 시작시 나이, 시력에 따른 총 치료 기간의 유의한 차이는 없었다.
약시안의 구면렌즈대응치 정도나 양안의 차이가 클수록 약시치료에 영향을 미친다는 여러 연구들도 있다[14-16]. Kirandi et al [11]은 높은 약시안의 구면렌즈대응치는 치료 실패의 중요한 위험인자가 된다고 하였고, Woodruff et al [17]은 두 눈의 구면렌즈대응치의 차이가 최종시력에 영향을 미친다고 하였다. 본 연구에서는 구면렌즈대응치 +3.0 D 이상의 원시값을 가질 때 총 가림치료 기간이 증가하였는데 이 것으로 약시치료 결과가 굴절부등의 정도와 관련이 있으며[18], 구면렌즈대응치가 높은 약시안에서 치료 기간이 길어진다는 것을 알 수 있다[10].
사시약시에 대해서는 내사시의 경우 내사시안의 중심와와 주시안의 강한 외측 반쪽 망막이 서로 경쟁하여 외사시보다 약시에 취약하다는 연구도 있으나[19], 본 연구에서는 내사시 환자의 원거리 사시각이 클 때 중간 성취 기간이 증가한 것 외에는 사시 종류나 크기에 따른 치료 기간의 차이는 없었다.
가림치료 이후 약시치료에 성공한 사시성, 굴절부등성 약시 환아를 대상으로 분석하였을 때, 약시안의 시력이 반대안과 동일하게 도달할 때까지 가림치료의 기간은 평균 23.15개월이었다. 가림치료 시작 후 중간시력까지 걸린 시간(half success duration)은 평균 9.98개월이었으며, 이는 총 가림치료 기간의 약 40%에 해당하였다. 특히 중간 성취 기간이 12개월 미만인 경우가 12개월 이상인 경우보다 통계적으로 유의하게 총 가림기간이 짧았다. 또한 치료 시작시 나이, 약시 정도는 치료 기간에 영향을 미치지 않았다. 굴절부등성 약시 중 구면렌즈대응치 +3.0 D의 중등도 이상 원시값을 보일 때 총 가림치료 기간은 평균 31.95개월로 길었으나 중간 성취 기간은 차이가 없었으며, 사시 약시에서는 내사시 환자에서 원거리 사시각이 20 PD 미만인 경우 총 가림치료 기간은 32.75개월로 차이가 없었으나, 중간 성취 기간은 9.88개월로 통계적으로 유의하게 짧았다.
본 연구의 한계점은 후향적으로 이루어진 연구로 의무기록을 바탕으로 하였으며, 가림치료 시간은 2-6시간 정도였으나, 환아가 실제로 가림치료를 시행한 시간에 대해서는 보호자의 진술에 의존하였다. 또한 약시 원인이 사시약시와 굴절부등약시가 혼합되어 있어 대상군이 다양하다. 그리고 Amblyopia Treatment Study와 같은 표준화된 시력표가 아닌, 스넬렌시력표를 기준으로 시력측정을 하였다는 점이다. 향후 이들을 보완한 연구가 필요하겠다.
결론적으로 약시 환아를 대상으로 가림치료를 계획할 때 이에 영향을 미치는 인자를 고려하되, 특히 나이가 7세 이상이거나 약시안의 시력이 0.5 이하이거나 사시각이 큰 사시약시인 경우에도 성공적인 약시치료를 위한 총 가림치료 기간이 유의하게 더 소요되는 것은 아니므로 보호자와의 충분한 반응을 통해 가림치료를 선택적으로 계획할 수 있겠다. 만약 시력호전이 치료 시작 후 비교적 초기인 12개월 이내에 보이기 시작한다면, 좋은 시력을 얻길 기대하면서 계속적인 가림치료를 권할 근거가 될 수 있을 것이라고 생각되었다. 또한 중등도 이상 원시 굴절률을 동반한 약시는 총 가림치료 기간이 길 수 있으니 중간 성취 기간에 도달한 이후에도 꾸준하고 계속적인 치료를 지속하는 것이 도움이 될 것으로 판단된다. 반면 적은 양 내사시를 동반한 사시약시는 초기 반응에서 시력호전이 좋은 듯해도 약시치료가 완전한 성공을 이룰 때까지 계속적인 가림과 추적 관찰이 필요할 것으로 생각되었다.
